The tenant's statement regarding the damage should be completed in your simpl.rent account. Find the appropriate policy and, after expanding the three dots, select "Fill out damage statement," then send the document to your landlord. If you are reporting the damage independently, choose "Make a claim" and follow the claims form.

The statement regarding the damage includes the following information:

• Date of occurrence/discovery of the damage.
• Damaged property (list all items that have been damaged).
• Description of how the damage occurred (what was damaged, where, and under what circumstances).
• Identification of who caused the damage (insured person on the policy, household member, housekeeper, insured person’s pet).
• Details of the person who caused the damage (name, phone number, email address, and PESEL number).
• Address of the rented apartment.
